Our commitment

Protecting the privacy and confidentiality of your personal health information is our top priority.

Every physician, nurse, technician, administrator, student, and contractor working under the NEOMO Health Group name is bound by this commitment. We collect only what we need, we share it only with those who need it, we keep it secure, and we account to you for how it is handled.

Who this covers

NEOMO Health Group (Northeastern Ontario Medical Offices) is a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA). This policy applies to NEOMO Medical, NEOMO Occupational Health, NEOMO Executive Health, NEOMO Skin, and NEOMO Travel, and to everyone working on our behalf.

How we handle your information

We collect personal health information in order to provide, coordinate, and follow up on your care, to administer and bill for our services, and to meet our legal and professional obligations. Wherever possible we collect it directly from you.

Access is limited to those who need it for these purposes. We do not sell your information, and we do not use it for advertising.

Your consent

When you come to us for care, we assume your consent for your care team to share information among themselves for the purpose of providing that care. Anything beyond that requires your express consent.

You can say no. You may withdraw or restrict your consent at any time by speaking to any member of our team or contacting our Privacy Officer. We will honour your instruction, and we will tell you plainly if doing so may affect the care we can provide. A small number of situations defined by law require disclosure regardless of consent, such as a court order or a risk of serious harm. In those cases we disclose only what the law requires, and no more.

Assessments requested by a third party

Where you are seen at the request of an employer, an insurer, or another third party, your clinical record stays with us. The requesting party receives an outcome, such as a fitness-to-work determination or confirmation that a required assessment was completed, not your medical history or diagnosis. We will explain the scope of the report before your appointment, and anything beyond that scope requires your written consent.

How we protect it

We use physical, technical, and administrative safeguards, including secured facilities, encryption and access controls, confidentiality agreements, privacy training, and auditing of access to electronic records. Service providers acting on our behalf are contractually bound to protect your information to the same standard.

If information in our custody is lost or accessed without authorization, we will contain and investigate the breach and notify you at the first reasonable opportunity, along with the regulator where required.

Your rights

You have the right to ask for a copy of your record, to ask us to correct it if you believe it contains an error, to withdraw consent for uses outside your direct care, and to complain about how your information has been handled.

Send access and correction requests in writing to our Privacy Officer. We respond within 30 days, or tell you in writing why we need longer. A reasonable fee may apply for copying and preparation, and we will give you an estimate first. Records are retained and then securely destroyed in accordance with the requirements of the College of Physicians and Surgeons of Ontario and applicable law.
